If you’ve ever swapped regular soda for diet, chosen sugar-free gum after a meal, or reached for a low-calorie dessert because you wanted to make a healthier choice, this is for you. A new study presented at the ESC Cardiology Congress in Germany this week has linked a popular artificial sweetener called xylitol to a higher risk of heart attacks and strokes. Xylitol is a sugar alcohol that has become a staple in the world of low-calorie and sugar-free foods. It’s in chewing gum, breath mints, sugar-free drinks, baked goods, and even toothpaste and mouthwash. The National Institutes of Health describes it as a low-calorie sweetener often used in greater amounts to replace sugar. For years, people have trusted it as a safe alternative, especially if they were watching their weight or managing diabetes. But the new findings suggest that higher levels of xylitol in the blood are associated with an increased risk of what doctors call major adverse cardiovascular events, or MACE. That’s a term that covers the most frightening outcomes: heart attack, stroke, and cardiovascular death. The researchers followed thousands of people over many years, and the results are prompting cardiologists and public health experts to ask whether this widely used sweetener is really as innocent as it seems. The study doesn’t prove that xylitol causes these events, and the researchers say more work is needed. But for a substance that millions of people consume every day without a second thought, it’s a wake-up call.

The study involved two large groups of people from different parts of the world, giving the findings extra weight. In the Canadian Longitudinal Study on Aging, researchers followed more than 17,700 participants over a six-year period. They measured the amount of xylitol in their blood and then watched to see who went on to experience heart problems. The results were striking: people with the highest levels of xylitol had a 57% higher risk of death, heart attack, or stroke related to cardiovascular events compared to those with the lowest levels. That is a substantial increase, especially for something that is supposed to be a healthier alternative to sugar. The second group, called the European Prospective Investigation into Cancer and Nutrition, or EPIC-Norfolk, involved participants from the United Kingdom who were followed for 30 years. In that group, those with the highest xylitol levels had an 18% greater risk of major adverse cardiovascular events. While the numbers are different, both groups showed a dose-dependent association. In other words, the higher the concentration of xylitol in the blood, the higher the rate of cardiovascular events. This pattern is important because it suggests a real relationship, not just a random finding. The researchers also adjusted for other factors that affect heart health, such as age, sex, body mass index, high blood pressure, diabetes, and cholesterol levels. Even after taking all of those into account, the higher risk remained. That makes the link harder to dismiss and leaves many experts feeling uneasy about how casually we consume products containing xylitol.

So why would a sweetener that tastes so much like sugar and has been approved by regulators be linked to heart attacks and strokes? The study’s presenter, Dr. Marco Witkowski from Charité University Hospital in Berlin, explained that previous research has already identified a plausible biological mechanism. Xylitol can enhance the ability of platelets to form clots. Platelets are the tiny blood cells that help your body stop bleeding when you get a cut. But when they become too sticky, they can clump together inside your arteries, forming blockages that lead to heart attacks and strokes. In essence, xylitol might be making your blood more prone to clotting, and that is exactly the kind of effect that can turn into a medical emergency. Dr. Witkowski also made a broader point about how we think about artificial sweeteners. “Artificial sweeteners are consumed by people who think they are healthier than sugar, and they are generally regarded as safe by regulatory agencies,” he said. “Our long-term findings in the general population highlight how little we know about the cardiovascular safety of xylitol and indicate that further studies are warranted, especially as the amount of xylitol in products continues to increase.” That statement cuts to the heart of the issue. Many people assume that if something is sold in stores and approved by agencies, it must be safe. But the reality is that some ingredients, especially those that have become popular more recently, may not have been studied for long-term effects at the levels people now consume. Xylitol is being added to more and more products, which means our daily exposure may be rising without anyone fully understanding the consequences.

Before you clear out your pantry, it’s important to look at the limitations of this study. The research was observational, which means it can show an association but cannot prove that xylitol actually causes cardiovascular events. There could be other factors at play that the researchers didn’t measure or couldn’t fully account for. The study authors themselves acknowledged that more research is needed to confirm the findings and to figure out what level of xylitol, if any, is safe. Another critical point is that the study did not establish whether high levels of xylitol in the blood necessarily reflect dietary consumption. Xylitol is not just an additive in processed foods. It is also produced naturally in the human body as part of glucose metabolism. So a person could have elevated xylitol levels without ever eating a sugar-free product. That makes it harder to say with certainty that xylitol from chewing gum or diet drinks is directly causing heart disease. The high levels could be a marker of some other metabolic issue that also increases cardiovascular risk. This is a classic challenge in nutrition research: trying to separate what people eat from the complex ways their bodies process it. Still, the dose-dependent association and the known effect on platelet clotting give the findings real weight. Even if this study is not definitive, it is a red flag that deserves serious consideration. It also reminds us that “generally recognized as safe” does not necessarily mean “proven safe for every person in every amount.” Regulatory approval often depends on short-term studies, and long-term risks can go unnoticed for years.

One cardiologist who was not involved in the study says these findings should be taken very seriously. Dr. Aseem Malhotra, a London-based cardiologist and public health campaigner, told Fox News Digital that the results are “disturbing” and that they align with what he sees in his own practice. “As a practicing cardiologist, I see many patients suffering from anxiety and palpitations that resolve as soon as they stop taking artificial sweeteners,” he said. This is a reminder that artificial sweeteners can affect more than just your long-term risk of heart disease. Some people experience immediate symptoms like a racing heart or a feeling of unease, and they often feel better once they cut these products out of their lives. Malhotra has a straightforward piece of advice for people who want to protect their heart: “Unsweeten your sweet tooth.” He believes one of the most important dietary changes anyone can make is to reduce their overall craving for sweetness. To do that, he advises all his patients to avoid all processed sugars and artificial sweeteners for six weeks. The goal is not to punish yourself, but to break the cycle of craving. When you stop feeding your body a constant stream of sweet tastes, your taste buds adapt. Eventually, you need much less sweetness to feel satisfied, or you might not miss it at all. It’s a challenging step, especially in a food culture where sugar and sweeteners are hidden in everything from salad dressing to bread. But Malhotra believes it’s worth it. He wants patients to rediscover the natural sweetness in whole foods like fruit rather than relying on processed substitutes that may come with hidden health risks. It’s also worth noting that Fox News Digital reached out to several food and beverage trade groups for comment, and they did not respond—which may say something about how uncomfortable these findings have made the industry.

At the end of the day, this study is not a reason to panic, but it is a reason to pause and think. If you occasionally chew a piece of sugar-free gum after a meal, you probably don’t need to fear for your life. The biggest risks appeared in people with the highest levels of xylitol in their blood, and those levels likely reflect regular, ongoing consumption rather than an occasional mint. Still, the findings are a reminder that “sugar-free” and “low-calorie” do not automatically mean “good for you.” Artificial sweeteners are powerful chemical compounds that interact with the body in complex ways, and we still have a lot to learn about their long-term effects. The safest approach might be to reduce your reliance on sweet-tasting foods and beverages altogether. That doesn’t mean you have to give up pleasure. It means shifting toward whole, unprocessed foods: water with a squeeze of lemon instead of diet soda, fruit when you want something sweet, and unsweetened tea or coffee instead of flavored drinks. If you do use xylitol-containing products for dental health—because it is a common ingredient in toothpaste and chewing gum that helps prevent cavities—talk to your dentist or doctor about the risks and benefits. For most people, the occasional use of xylitol is probably not a major problem. But if you have heart disease, diabetes, or other cardiovascular risk factors, it might be wise to choose products that don’t contain xylitol and to keep an eye on emerging research. The bigger message from this study is one of humility. Our food environment is filled with products that promise convenience and health, but science is constantly catching up. What we know today may change tomorrow. In the meantime, listening to your body, eating real food, and being thoughtful about what you consume is the closest thing we have to a guarantee.
